O tromboembolismo venoso (TEV) nas suas formas mais frequentes -trombose venosa profunda (TVP) e embolia pulmonar (EP) -é um problema bem reconhecido nos doentes hospitalizados como a primeira causa de morte intra-hospitalar evitável e como gerador de elevada morbilidade aguda e crónica1. A aplicação das recomendações existentes sobre a prevenção do TEV assume assim grande importância nos doentes mais vulneráveis -os doentes cirúrgicos2. O estudo europeu ENDORSE, realizado na década passada e que incluiu nove hospitais portugueses aleatoriamente selecionados, revelou que, nos nossos hospitais, 52,7% dos doentes internados tinham risco de TEV de acordo com as definições do American College of Chest Physicians (ACCP) (68,9% nos doentes cirúrgicos e 38,5% nos doentes médicos); 58,5% dos doentes em risco tiveram uma profilaxia de TEV adequada, mas também mais de um terço dos doentes internados, médicos ou cirúrgicos, efetuaram profilaxia do TEV sem haver indicação por não terem risco para TEV3,4. O progressivo reconhecimento desta situação conduziu à realização de várias iniciativas por diferentes protagonistas nos anos mais recentes. A Direção Geral de Saúde (DGS) incluiu na norma sobre a Cirurgia Segura, Salva Vidas, a verificação perioperatória de indicação para tromboprofilaxia e produziu uma norma específica para a cirurgia
